Family Outreach Specialist

Department: Family Support Services
Location: Des Moines, IA

Purpose:

Assists families in connecting with community-based healthcare, human services, and community resources through 1st Five via enhanced care coordination and referrals.

Responsibilities:

  • Collaboratively develops and maintains accurate and thorough resource directories and databases of community-based family and child health organizations, programs, services and experts.
  • Conducts outreach efforts to ensure information about program benefits, services and resources is marketed to appropriate audiences.
  • Contacts clients by phone, email, and/or through home visits to assess needs and successfully link families to effective services related to healthy mental development, and assists families with scheduling appointments, finding interpretation services, childcare, transportation and other resources as necessary.
  • Uses a family-centered approach in advocating for, educating and supporting families as they select and use effective resources for their needs.
  • Provide follow up to referring primary care practices regarding referral activities and status.
  • Performs accurate and timely data entry and documentation.
  • Organizes, analyzes and writes periodic and ad hoc reports for 1st Five – Healthy Mental Development Initiative, and VNS of Iowa.
  • Ensures that project evaluation/assessment tools are completed within the scope of the programs.

Qualifications:

  • Family Outreach Specialist: Bachelor’s degree in social work, sociology, psychology, health education or nursing
  • At least 1 year of experience providing home visitation or care coordination services 
  • Experience working with diverse communities preferred
  • Ability to communicate effectively, both verbally and in written English; bilingual in Spanish preferred 
  • Aptitude for technical writing
  • Ability to identify family strengths and needs for family-centered support
  • Knowledge of community resources
  • Analytical skills to interpret information
  • Ability to work independently and exercise sound decision-making 
  • Strong keyboarding and computer skills with the ability to learn work-related software; knowledge of Microsoft Office, including Word, Excel and Outlook; knowledge of database use and management
  • Mandatory Reporter of Child and Dependent Adult abuse training certification (within 30 days of hire)
  • Must have valid driver's license, auto liability insurance and reliable transportation if driving is required
  • Successful completion of pre-employment physical, drug screen, 2-step TB test and background check

Working Conditions:

  • Physical requirements: Light work, sitting most of the time, exerting up to 20 pounds of force frequently, and/or a small amount of force constantly to move objects 
  • Physical activities: Kneeling, Reaching, Pushing, Talking, Hearing, Repetitive motions, Standing, Stooping, Walking, Lifting, Fingering (picking, pinching, typing, etc.), Grasping (applying pressure with fingers and palm), Visual acuity (field of vision for analyzing data, viewing computer, using measurement devices, operating machines, determining accuracy or neatness, and/or making general observations)
  • Environment: Extreme temperatures, Hazards (chemicals, infectious diseases), Atmospheric conditions (fumes, odors, dusts, mists, gases)
  • Time: Ability to be flexible with a varying schedule and programmatic requirements
  • Travel: Required between locations
